Copy of letter from IRS awarding tax exempt or non profit status

Limited Liability Company

Operating agreement naming managing members for banking purposes (if one exists)

Certificate of formation stamped by Secretary of State 

Business License

Non Profit Organizations or clubs

Charters, By-laws or meeting minutes naming officers/representatives or a document issued by any type of government office, or a state and regional office of the same entity, recognizing the local organization.
